Eligibility
Must be Native American and a senior age 50 and older. Family caregiver and respite services must be age 18 and older and provide unpaid care to someone age 18 and older.

Description
Provides case management and information & referral services for seniors ages 50 and over and their families. Also provides senior farmers market voucher for those living in the reservation. Assesses client's needs and matches them to services available. Provides respite and family caregiver support services, COPES, and Medicaid Personal Care Case Management services.
