Provides case management and helps arrange for long-term care needs. Helps to develop a plan of care by performing a comprehensive assessment of the client's needs and resources. The case manager stays involved to assure that the plan is implemented properly. Provides assistance when a caregiver is unable to handle increasing care needs, if a family wants to know what options they have, or because a person needing care wants to talk to someone about their options.
