Provides case management, care coordination and referral to community-based programs. Maternal Child Outreach Team - MCOT Works to reduce high rates of infant mortality in Pierce County. Connects newly pregnant mothers to health insurance including Medicaid, smoking cessation and other support services. Maternity Support Services - MSS Promotes healthy pregnancies and babies through nurse home visiting. Provides preventative health and education services to women who are enrolled in Medicaid. Nurse Family Partnership - NFP Provides nurse home visiting services for first-time pregnant low-income women. Improves birth outcomes, child health and development, and economic self-sufficiency of families. Referrals must be received by 26th week of pregnancy.

Serves those who are pregnant or have children under 1 yr old that receive medical insurance through the State of Washington. Provides professional in-home visits by a staff of licensed nurses, counselors, and dietitians for low income or at-risk pregnant women who are facing barriers to having a healthy pregnancy. Works with women during pregnancy and, depending on her needs, into her baby's first year of life. Each client is provided with one-on-one counseling and education, in her home or place of residence, to help her have a healthy pregnancy and take needed steps to prepare and provide for her family. Also assists with resources life skills classes, community events, vital needs (such as diapers, car seats/cribs and food), job training, and many other services and resources to help women succeed. Moms can also continue to attend events and gatherings, even after they are not currently in the program. May also provide information on Safe Sleep and access to cribs to those receiving services.

Maternity Support Services and Infant Case Management are preventive health and education services to support healthy pregnancies and healthy babies. Prenatal support can help prevent complications and inform women about important steps they can take to ensure a healthy pregnancy, birth, and baby. We offer social support, counseling, nursing, parent education, and nutritional services.

A program that provides visits from specially trained nurses for women who are having their first baby. Nurses are able to visit at home or another location that works for the client throughout their pregnancy and up until the child turns the age of 2.
