All Recovery Outreach Specialists are trained Recovery Coaches and Certified Peer Counselors in long term recovery from SUD and/or have lived similar experiences, i.e., homelessness, incarceration, involvement with Department of Corrections or law enforcement, or close connection to SUD during their life. Helps families and individuals connect to available resources. Resources available include housing, health insurance, child care, food assistance programs, and other kinds of services. The services available differ by area. Services are provided at eight regional hubs. Local care coordinators work with a person to help them apply for programs such as unemployment benefits, subsidized housing or child care, food assistance programs such as SNAP or WIC, Apple Health, and more.

Provides complex medical care for older adults who are frail and have multiple medical conditions. Offers memory loss and dementia assessments, care coordination, disease management and health education as it pertains to geriatric care.

Helps recently arrived refugees remove barriers toward achieving economic independence and integration into their new communities.

Family Behavioral Health is an integrated, intensive service area within CCS providing care using a "Wraparound" (Individualized and Tailored) approach. Services are designed for children/youth and families with multiple and complex needs, severe histories and who tend to be involved with multiple child-serving systems.   Services include:   Pediatric Integrated Care - Wraparound with Intensive Services (WISe)  -  Behavioral Rehabilitation Services

Offers intensive mental health care and team-based case management for youth with complex needs and their families. Looks for community-based options to support multi-system-involved children and youth. Staff members trained in team facilitation meet with a family, listen to their story, help the youth build a team of natural and formal supports, and guide this team through the wraparound process. Team members include natural supports (such as family, friends, and religious leaders) and the professionals who work with the family (such as counselors, schools, Child Protective Services and probation officers).

Provides individual, group, and family therapy; as well as brief intervention treatment, rehabilitative case management, and crisis support services.

Medicaid program provides assistance, case management, and assessment to seniors and adults with disabilities to access specific long term care plans including in-home services, adult family homes, adult day centers, assisted living, nursing home placements, and respite care programs for family members, guardians and caregivers of dependent adults.

Provides confidential support to empower clients through respect, resources, and relationships. Works with clients by providing holistic resources, aftercare, information and referrals for financial aid, educational and GED assistance, counseling, trauma support, and connects clients with local medical services, legal aid, rehabilitation, safe housing, and more.

Helps individuals with serious and complex health care challenges to navigate large health care systems and find the resources they need. Improves coordination of medical and social services needs such as long-term care, mental health, medical specialists, and housing. Assists with such activities as: transition planning when leaving a hospital or care facility; referral to community and social supports, such as transportation and food resources; and coordinating better communication between your medical providers, family, and other caregivers.

Provides a network of care providers who offer comprehensive, up-to-date resources to assist Sickle Cell carriers and their families with: - Access to Washington state's most dedicated sickle cell specialists - physicians, nurses, social workers, genetic counselors and more. - Opportunities to learn about new ways to prevent and manage pain, to work with teachers and schools, and to otherwise support a child's health, growth and development. - Social activities and the chance to meet and learn from other families who are living with sickle cell disease. - Regular updates on the latest research, community activities and other resources. Provides assistance in patient-provider communication issues.

A free program that matches parents with a trained Parent Educator (PE) who will meet in the parents home or a convenient location like your local library or park. The PE is an experienced person who can answer questions about pregnancy or being a parent and can help connect parents to services for them and their children. Services support participants that have serious chronic health conditions and social service needs. The program provides care coordination with medical and behavioral health as well as long term services and support through a person-centered action plan at no cost to the participant. Services include provider management, appointment assistance and connecting with community resources.

Provides independent living planning to empower people to live independently in their community. Assists with identifying and navigating complex social service systems, housing, transportation, preparing for employment, identifying accommodations, personal assistance management, applying for social security, benefits planning, and advocacy. - Advocacy: works alongside individuals with disabilities to plan and carry out strategic approaches to influence positive change in the community. Represents and advocates for the rights and needs of individuals with disabilities in order to advance the empowerment, inclusion, and wellness of all persons with disabilities. - Emergency Preparedness: Provides emergency preparedness for disabled individuals by creating a plans in case of a disaster. Helps with fire drills, emergency planning, connection with local EMS, and building an emergency kit. - Assists individuals or veterans who are needing to find caregiver and helps with making sure their caregiver gets paid. Must be referred by the VA, if they are a veteran; if they are a civilian, then they'll get help finding a state-funded caregiver. - Has a Covid -19 Advocate that works with individuals with disabilities to help them get a schedule for vaccines, as well as a covid-19 peer support group. - Assists with Power Action Systems (PAS program), which helps individuals with disabilities be able to get modifications to their household to make it more disability accessible.

Offers substance use assessments and support through Foundational Community Supports. Drug and Alcohol Assessments Assessments are offered in-person, through tele-health, and a mobile assessment. Community Navigators Provides Foundational Community Supports to those experiencing homelessness. Services can include providing navigation and support to help remove barriers around housing and employment.

Offers needs assessment and support services for older Mercer Island adults and adult children of older adults. Services are mostly office based; home-based services are available by appointment.

The Senior Housing Stability Program provides navigation support services to seniors who are homeless or whose housing has become unstable. An Individualized Action Plan is developed to prevent homelessness or remedy the housing instability.

Provides obstetrics and additional supports to help low-income women have a healthy pregnancy and until baby turns 1 year old. Includes -  Individualized visits at the clinic -  Home visits postpartum for health checks as well as information about feeding, bathing, newborn care and behavior and breastfeeding support. -  Additional home visits for special concerns -  Visits with nutritionist, social workers, community health workers -  Referrals to WIC, health insurance and other services

Helps households on the Housing Choice Voucher (HCV) Section 8 program work toward financial independence by providing one-on-one coaching. Supports families in achieving goals related to employment, education, homeownership, health, and more.

Provides bilingual, bi-cultural (Spanish/English) support for residents who are unable to, or unsure of how to, access resources on their own or problem-solve issues that present with barriers moving forward. The program provides clients with interpretation services so that they can access resources such as WIC, legal aid, shelters, etc. Helps the client break down their needs into manageable steps, making problem solving less overwhelming and providing the skills necessary for them to navigate service systems in the future. Also offers a diaper bank.

Provides case management services for refugee and asylee families in need of support, specializing in assistance for individuals with medical needs.

Provides housing case management, financial counseling, and limited, emergency rental assistance to individuals and households who are at imminent risk of homelessness.

Provides preventative health services to help people have a healthy pregnancy.   Offers family-centered obstetrical care, including delivery at Swedish - First Hill. Provides individualized education and counseling services for women and their partners before and after birth.   Offers the Parental Support Services case management program for low-income people who receive state medical assistance.   Team includes a nurse, nutritionist and a social worker.

Helps homeless veteran individuals and families move toward housing stability. Program participants may be referred to employment and training programs, financial and community resources, and help accessing benefits.   Financial assistance may be available for rental deposits and other household needs.
