Works with the individual to explore options that can help them live independently longer in a manner that is safe and meaningful to them. An options counselor can meet with the older adult one on one to: - Develop a personalized action plan - Assess and refer to additional resources - Provide limited assistance in filling out paperwork - Can provide limited phone-based advocacy to connect client to resources During visit CANNOT provide: - Housework - Errand running - Personal care services - Transportation - Legal representation or advice
